On June 29, 2019, I mailed my Body Donor Registration Form to Professor Krabbenhoft, but I don’t recall receiving, as of July 22, any confirmation that your program has received and reviewed the form. I believe I am in good health at this time, but I am trying to put a few things in good order and would appreciate a note (e-mail is okay) letting me know that your program is looking forward to seeing me eventually.
